HIPAA Notice

The boundary between this informational website and the clinical, HIPAA-covered care delivered at our parent practice.

1. This site is not a HIPAA-covered service

amavitaresearch.com is the public, informational, and recruitment website of Amavita Research Services, LLC (Amavita Research). This Site does not collect, store, or transmit Protected Health Information (PHI) as defined by the Health Insurance Portability and Accountability Act (HIPAA).

2. What you submit through this Site

Patient and sponsor contact forms collect only basic identifiers (name, email, phone, language preference, trial of interest, and any free-text message you choose to send). Please do not include sensitive medical details in form messages. If you do include them, that information will be handled under our Privacy Policy and not as PHI under HIPAA.

3. Where PHI is collected

PHI is collected only at clinical visits at the parent practice, amavita Heart and Vascular Health®, which is a HIPAA-covered entity. PHI handling at the practice is governed by its own Notice of Privacy Practices, which is provided to patients at the first clinical visit and on request.

4. Clinical research records

For patients enrolled in a clinical trial, study records are maintained per the trial protocol, the informed-consent document, ICH-GCP E6 (R3), and applicable federal regulations (45 CFR 46, 21 CFR 50/56/312/812). Study sponsors, monitors, IRBs, and regulatory authorities may access de-identified or coded records as authorized by your written informed consent.

5. Common Rule and IRB oversight

Participation in any clinical trial is voluntary and requires written informed consent reviewed by an Institutional Review Board (IRB) before any study procedure. You may withdraw at any time, for any reason, without affecting your medical care at the parent practice.
